Notes to applicant

1. The requirements stated in this application form are subject to change, to adapt to the continual development and evolvement of the electricity industry. Until a licence is renewed, you are advised to check with the Authority from time to time regarding the latest licensing requirements.
2. 2. If an application for the licence renewal is granted, you will need to pay an annual licence fee on or before 30th April of the year following the expiry of your current licence and in each subsequent year.

Completion Instructions

1. The estimated time to complete this form is 1 hour and 10 minutes.
2. You will need the details of your company shareholders/directors; reasons for seeking the licence renewal; summary of any past breaches of licence conditions and/or regulatory action taken by EMA against it and any proposed amendments to the conditions in the current Electricity Licence.
3. Print clearly or type all information. If there is insufficient space in the form, please provide additional information in an attachment.
4. Please submit the following list of documents together with the application:
   (a) Certified true copy of the Applicant's certificate of incorporation;
   (b) Certified true copy of the Applicant's Memorandum and Articles of Association; and
   (c) Certified true copy of relevant ownership documents, including documents lodged with the Accounting & Corporate Regulatory Authority, ACRA (or equivalent authority in relevant jurisdiction) showing that the final shareholder structure is in place. Include names and addresses of significant shareholders; and
   (d) Certified true copy of relevant ownership documents, including documents lodged with the Accounting & Corporate Regulatory Authority, ACRA (or equivalent authority in relevant jurisdiction) showing that the final shareholder structure is in place. Include names and addresses of significant shareholders.
5. All attachments shall be submitted as zipped files and appended in the space "Section III. Upload Attachments" (maximum 1MB) or in hard copies (TWO, 2 copies) to be sent to the address given above, to the attention of "Deputy Director, Economic Regulation & Licensing Branch, Regulation Division."
6. If any information provided by the Applicant is of a confidential or commercially sensitive nature, please identify such information in a separate document by noting the relevant sections and citing reasons why the information is considered confidential or commercially sensitive.
